TT is prolonged in cases of low levels of fibrinogen, when fibrinogen function is impaired, and in the presence of heparin, fibrin/fibrinogen products, and thrombolytic agents like streptokinase. • Fibrinogen (factor I) is a soluble plasma protein that is instrumental in the normal coagulation process. Following trauma or injury, fibrinogen is converted to an insoluble fibrin clot by two stage process. In stage one; thrombin cleaves fibrinogen to form a fibrin monomer. In stage two; these fibrin monomers aggregate to form the soluble fibrin polymers that are recognized as the end product in thrombin clotting assays. • The thrombin Time (TT) is a routine test to monitor heparin therapy. PRINCIPLE • The thrombi time is a simple test to screen conditions that can interfere with the conversion of fibrinogen to fibrin. A low potency thrombin is undiluted plasma and clot formation is timed. MATERIALS • Two way needle • Citrated evacuated tube • 0.5 ml serological pipette • 0.6 ml serological pipette • Tissue paper • Timer • Thrombin Time Reagent • 37oc water bath • Wasserman tube • Centrifuge PROCEDURES 1. Pipette into prewarmed test tube 2. Plasma/ control 0.2 ml 3. Incubate 3 minutes at 37oc 4. Add thrombin time reagent 0.1 ml 5. Start timer with addition of reagent 6. Record time required for clot formation • Reference Range : 8- 14 seconds •
