We will only describe the insertionapophysitis of the long biceps. ➢ Apophysitis of the scapular insertion of the long biceps ❉ Etiology This pathology can manifest itself in an acute form during a brutal action (blocked shot), ormore often in its chronic form (excessive shooting training). ❉ Diagnosis . Physical examination The gradual onset of pain is located at the insertion of the tendon on the bone, it is exacerbated by the digital pressure exerted vertically "through"the deltoid, still underdeveloped at this age, and the thwarted contraction of the long biceps.. Complementary exam The radiography, which is quite evocative during the puberty period,is often silent before this age. Practiced in comparison with the opposite side, it can show ablurred aspect of the part superior to the glenoid, more rarely a displacement of the bonyprocess in the axis of the muscle. Bone scintigraphy is of little interest (the fixation ofisotopes on the growth cartilages masks the lesion). ❉ Treatment Treatment includes: . ● Daily cryotherapy (2 to 3 sessions), lasting minutes. We will take advantage of the vasodilation caused by cryotherapy to apply anti-inflammatory ointment. . ● A stoppage of the movements causing the apophysitis. This stop will only be selective, the child being able to take advantage of this rest time to perfect histechnique, his tactics or to work other anatomical regions. ● Anti-inflammatory and analgesic physiotherapy combined with physiotherapy, the exclusion of ultrasound, contraindicated on growth cartilages. . ● Oral anti-inflammatory treatment is only justified in acute forms. ● The resumption of activity will be carried out according to clinical criteria and radiology, emphasizing the need to modify the training responsible for the lesion.
